Important Information for our Patients

New Patient Registration
To register as a new patient, please call the office at 360-943-9519 and select option 2 to be connected to a staff member who can help. If you reach our voicemail, please leave a message and we will aim to return your call within 24 hours (up to 72 hours at the latest). We appreciate your patience and understanding. Please complete either the Adult Registration form or the Pediatric Registration form (up to age 18) as appropriate. Please print and bring to your first appointment, arriving 15 minutes ahead of your appointment time.
It is possible to complete the forms in person before your appointment, but you MUST arrive 30 minutes early to do so. Please also remember to bring your insurance card and photo ID to your first appointment.

We have appointments available Monday – Thursday from 9:00am – 5:00pm and Friday 9:00am – 3:00pm. Our current reception hours are 9:00am – 5:00pm Monday through Thursday and 9:00am – 12:00pm on Friday.
Whole Health Naturopathy has a new medical records system and all appointments should now be made through it. As an existing patient, you are now able to log onto the portal for easier scheduling. Please ensure that you keep track of this portal link and your sign-in account information.

Telehealth Appointments
Telehealth appointments are still available at Whole Health Naturopathy. Once you’ve scheduled an appointment, the links below will bring you to your appointment with your doctor at the scheduled date and time:
Important Policies
- We accept Regence, Regence Uniform, Premera, Lifewise, Kaiser Options plan only, and First Choice Health Plans as well as cash pay. All copays, balances and cash payments are due on the day of service.
- It is the patient’s responsibility to make sure we are in your insurance network, to be aware if you have a deductible and to make payments in a timely manner. Appointments will not be made if there is a balance of $100 or more.
- Cancellations within 24 hours of your scheduled appointment will incur a $75 cancellation fee
- All intake paperwork must be completed, signed and returned prior to scheduling the initial appointment. This ensures time to receive, upload and make sure all documents needing signatures are signed. This also gives the provider enough time to prepare and review your history prior to seeing you. If it is not received in time, you will risk a cancellation fee and rescheduling.
Masking Policy
Masks are currently optional at Whole Health Naturopathy. However, if you are visiting for a sick appointment, we kindly ask that you wear a mask to help protect our team and other patients.
